Specialized Care Program
Daily Telehealth Monitoring is available to patients with chronic cardiac and respiratory illnesses if they meet the screening criteria. This monitoring enables patients to measure and transmit important clinical information, such as blood pressure, heart rate, oxygen saturation, weight, and ECG readings back to the home health clinician for interpretation.

The Telehealth units are programmed to specifically meet the needs of the individual patient, and provide a weekly trend report to the physician. If the monitor picks up a response that is outside the desirable range set for the patient, an ALERT is activated and reviewed by a nurse. The patient is contacted immediately to discuss the finding and learn more the patient’s status. A nurse may make a home visit or the physician will be called depending on the need of the patient

Criteria for Admissions
Skilled Care—Medicare federally funded program mandates that admission to this agency can only be made upon the recommendation of a physician, based upon the needs of the patient, homebound status and type of skilled care and services required. Commercial insurance benefits and guidelines may vary from policy to policy.

Payment Options:
We accept payment for services from Medicare, Workers’ Compensation, private insurance or private pay. Florida Palliative Home Care will assist you in assessing your insurance coverage. Be assured that prior to the start of care, you will be informed of your financial responsibility.
